Registration packets may be emailed to the clinical director at [email protected], brought to your appointment, or mailed to our office.
For patients of community midwives and physicians credentialed with our birthing center, when we receive your packet, we will contact you and your midwife within 5 business days to confirm registration with our birth center and discuss any required financial deposits or needed information. Final birth center approval for delivery will require receipt of your prenatal record and any necessary deposits on care by 37 weeks.

Mobile Midwife Registration
All birth center patients must also register with mobile midwife, our electronic medical record for delivery. Please follow this link to complete this registration by 37 weeks. Please input your basic demographic information, current and past pregnancy information. Complete health intake does not need completed and no forms need signed within the mobile midwife system. Press SUBMIT when completed to save your registration with the system.
